Position Summary:

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our home health team. The LPN will provide direct patient care in the home setting under the supervision of a Registered Nurse (RN), following the plan of care and ensuring high standards of clinical excellence.

Key Responsibilities:

• Provide nursing care to patients in their homes.
• Assist with implementing and following individualized care plans under the supervision of an RN.
• Educate patients and caregivers on disease processes, medications, and self-care techniques.
• Document patient care accurately and timely in compliance with company policies and regulatory requirements.
• Collaborate with interdisciplinary team members to ensure continuity of care.
• Respond to patient needs and emergencies as appropriate.

Qualifications:

• Current and unrestricted LPN license in the state of Oklahoma.
• Minimum of 1 year of nursing experience (home health experience preferred but not required).
• Current CPR/BLS certification.
• Reliable transportation and a valid driver’s license.
• Strong clinical, communication, and organizational skills.
