Job Summary:

The Home Health RN is responsible for delivering skilled nursing care to patients in their homes, coordinating with physicians and interdisciplinary team members, and ensuring patient-centered, quality-driven outcomes.

Responsibilities:

• Provide direct nursing care to patients in their homes according to established care plans.
• Conduct comprehensive assessments and update individualized care plans.
• Administer medications and treatments as prescribed.
• Educate patients and families on disease processes, medications, and self-care techniques.
• Communicate effectively with physicians, care team members, and patients’ families.
• Document all care and interventions accurately and timely in compliance with regulatory requirements.
• Participate in quality improvement initiatives and staff meetings.

Qualifications:

• Active, unrestricted Registered Nurse (RN) license in Oklahoma.
• Minimum 1 year of nursing experience (Home Health experience preferred).
• Current CPR/BLS certification.
• Valid driver’s license and reliable transportation.
• Strong critical thinking, organizational, and communication skills.
• Comfortable working independently in a home setting.
