$2500 sign on bonus! Linwood Village is a leading long-term care and skilled nursing facility dedicated to providing exceptional care and support to our residents. With 67 licensed beds, we focus on delivering personalized, high-quality care in a warm and welcoming environment. We are looking for dedicated and compassionate individuals to join our team and make a meaningful difference in the lives of our residents.
We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to provide high-quality care to our residents. The LPN will be responsible for delivering direct patient care, monitoring patient health, and collaborating with other healthcare professionals to ensure optimal patient outcomes. The ideal candidate will have a strong commitment to patient care, excellent communication skills, and the ability to work effectively in a team-oriented environment.
Key Responsibilities:
- Provide direct patient care, including administering medications, wound care, and treatments as prescribed by physicians.
- Monitor and document patients' vital signs, physical and emotional status, and response to treatments.
- Assist with the development and implementation of individualized care plans in collaboration with registered nurses and other healthcare providers.
- Perform routine assessments and report changes in patients' conditions to the healthcare team.
- Educate patients and their families about health conditions, treatment plans, and self-care techniques.
- Ensure proper infection control practices and maintain a clean and safe environment for patients.
- Assist with activities of daily living, including bathing, dressing, and mobility support.
- Participate in interdisciplinary team meetings and contribute to care planning and problem-solving.
- Adhere to facility policies and procedures, as well as state and federal regulations, including HIPAA compliance.
Qualifications:
- Current and valid Licensed Practical Nurse (LPN) license in Oklahoma.
- High school diploma or equivalent; completion of an accredited LPN program.
- Previous experience in long-term care or skilled nursing preferred but not required.
- Strong clinical assessment and critical thinking skills.
- Excellent interpersonal and communication skills, with a patient-centered approach.
- Ability to work collaboratively in a team environment and adapt to changing patient needs.
- Physical stamina and strength to handle the demands of the role, including lifting and assisting patients.

We are seeking a compassionate and skilled Certified Nurse Aide (CNA) to join our healthcare team. The CNA will provide direct care to patients, assisting with daily activities and ensuring their comfort and well-being. The ideal candidate will have a strong commitment to delivering high-quality care and possess excellent communication and interpersonal skills. We have opportunities for night shift 12-hour rotation.
Key Responsibilities:
- Assist patients with daily living activities, including bathing, dressing, grooming, and toileting.
- Monitor and record patients' vital signs, such as blood pressure, temperature, and pulse.
- Provide support with mobility and transfers, including using wheelchairs, walkers, and lifts.
- Assist with feeding and hydration, and monitor patients for any dietary needs or restrictions.
- Observe and report any changes in patients' conditions or behaviors to the nursing staff.
- Maintain a clean and safe environment for patients, including changing bed linens and cleaning rooms.
- Provide emotional support and companionship to patients, promoting their overall well-being.
- Follow infection control protocols and maintain patient confidentiality in compliance with HIPAA regulations.
- Collaborate with other healthcare professionals to ensure coordinated and effective patient care.
Qualifications:
- High school diploma or equivalent required.
- Valid Certified Nurse Aide (CNA) certification from an accredited program.
- Previous experience in a healthcare setting preferred but not required.
